So Ji Sub and Shin Hyun Joon make their small-screen comebacks in the 2009 SBS drama Cain and Abel. Taking its title from the biblical story of brotherly strife, this thrilling mini-series details the contentious relationship and rivalry between two brothers who clash over love, family, and work. Starring in his first major TV drama since I'm Sorry, I Love You, post-army So Ji Sub faces off against movie star Shin Hyun Joon, also returning to Korean television for his first drama since Stairway to Heaven. Directed by Kim Hyung Suk (Surgeon Bong Dal Hee, Lovers in Prague), Cain and Abel attracted a lot of buzz from the get-go with its big budget and big names, and the story proves to be a great acting showcase for the two dashing leading men. Han Ji Min (Capital Scandal) and Chae Jung Ahn (1st Shop of Coffee Prince) co-star as the women in their lives.

Based on the biblical story of Adam and Eve¡¯s first two sons, Cain and Abel is about Cain¡¯s jealousy towards his brother Abel. Lee Cho In is a very gifted doctor who has everything that he wants whereas his older brother, Seon Woo, is jealous of all the attention that Cho In receives. Seon Woo blames his brother for taking everything good in his life away from him. Seon Woo blames Cho In for getting their father¡¯s love, getting more recognition as a doctor, and for stealing the woman he loves.
After serving military service in South Korea, So Ji Sub is back for his role as Lee Cho In, a gifted doctor, in "Cain and Abel" and does not disappoint. The title has biblical reference to Adam and Eve's first sons, Cain and Abel. Cain is jealous that God accepts the sacrifice of Abel's over his and murders Abel, committing the first fraticide. Shin Hyun Joon plays Lee Seon Woo (Cain) a neurosurgeon who is jealous that his younger brother, Lee Cho In (Abel), has their father's support, along with the love of his ex-girlfriend Kim Seo Yeon (Chae Jung Ahn). The drama revolves around the contention in the prestigious hospital, with one faction led by Seon Woo and his mother to build a Neurology Center while Cho In supports the building of the Emergency Center. While on a visit to China, Cho In meets Oh Young Ji, a North Korean refugee (Han Ji Min) who pretends to be his tour guide but is secretly hired to follow him by his evil step-mother's hooligans. Oh Young Ji is touched by his kindness though and ultimately tries to warn him. To prevent Cho In as an obstacle to the vote for the Neurology Center, he is kidnapped and shot in the desert. He is saved by North Korean militant refugees in China but loses his memory and becomes Oh Gang Ho, the younger brother to Oh Gang Chul, who also happens to be Young Ji's brother. What happens when Cho In finally gets back to South Korea and regains his memories to find that it's his beloved brother who abandoned and tried to kill him?
In the biblical sense, this drama's foundation is based upon the story of Cain and Abel. Cain was a farmer while Abel was a shepherd, and in short, rooted in jealousy, Cain ends up killing his brother Abel...

In this drama, Lee Cho-In (So Ji-Sup) most resembles Abel in that he is sincere, loved by his father, and is a successful doctor. His older brother, Lee Sun-Woo (Shin Hyun-Joon), will take the characteristics of a jealous Cain. It is through this relationship where the jealousy of love, acknowledgment, and even one another's love for a woman that sparks this drama.

A hospital owner is killed in a car accident and his pregnant wife falls into a coma. His partner and friend adopts the baby who is raised in an incubator. The baby grows up with the son of his foster parents, who are now the sole owners of the hospital. The two brothers grow up to become prominent neurosurgeons.

Despite the brotherly affection between the two boys whilst growing up, Seon-u's affection for his younger brother turns into bitter jealousy when he realizes his father's desire to hand over the management of the hospital to his adopted younger brother, Cho-in. This jealousy runs out of control when he realizes that Cho-in has also stolen the heart of his love, Seo-yeon. Seon-u plans the murder of his younger brother and pays a killer to do the job. Miraculously surviving the attempted murder, Cho-in plans on his own revenge.
